Waiver
By completing this form, participant and family, on behalf of participant, his/her heirs, assigned next of kin, agrees to assume all risk and responsibility involved with participation in the physical activities. Further, all aforementioned parties agree to fully release Southern Maine Athlete Academy, LLC (as well as any of it’s owners, employees, volunteers, or other authorized agents, including independent contractors), and Jeremy Longchamp from any and all liability, claims, and/or litigation resulting from training programs and the physical activities, even if caused by the negligence, gross negligence, intentional acts, or omissions, and/or any other type of fault of Jeremy Longchamp or Southern Maine Athlete Academy, LLC, it’s owners, employees, volunteers, or other authorized agents, including independent contractors. Further, signee gives Southern Maine Athlete Academy, LLC, permission to photograph, record, and post participant to any public or private media outlet, and grants, in perpetuity, a non-exclusive, royalty free worldwide license to copyright all media without limitations.
